Steps to Get a Work Permit
- Obtain a job offer
- Your employer must complete the Promise of Employment section.
- They must include:
- Business name
- Job duties
- Work hours
- The employer must sign the form.
- Doctor’s certificate (ages 14–15 only)
- A physician must complete the Physician’s Certificate of Health section on page 2 of the work permit application.
*A regular health physical cannot be accepted.
- A physician must complete the Physician’s Certificate of Health section on page 2 of the work permit application.
- Parent/Guardian and minor signatures
- Both the parent/guardian and the minor must sign on page 2.
These signatures do not need to be witnessed by the issuing representative
- Both the parent/guardian and the minor must sign on page 2.
- The student brings completed printed application to the Superintendent’s Office
Does not need to be accompanied by their parent/guardian- The application must already be completed.
- We are unable to accept the completed promise of employment that is shown to us in the student’s email on their cell phone. The application that is presented to us must be printed and all fields completed depending on the age of the student.
- Bring proof of age, such as:
- Birth certificate; Driver’s license; or any official documentation showing their date of birth
- The application must already be completed.
- Receive the work permit
- The school representative issues the official permit.
- The student returns the permit to their employer to keep it on file.
Important Notes:
- Students who currently attend Martha’s Vineyard Regional High School should complete the work permit process at the high school. Students should check with the main office for work permit hours.
- Work permits are job-specific and do not transfer between employers.
- If you get a second job, you must complete the process again.

- Monday–Thursday: 9:00 AM – 3:30 PM
- Friday: 9:00-3:15 PM
Summer
- Monday–Thursday: 9:00 AM – 1:00 PM
